Customize Content Approval Questionnaire
This form is designed to be completed after the Process Mapping Session. It helps inform your Phase 2 build-out and supports specific tasks noted in the Project Board that comes with The Setup OS.
The CAQ collects key info from your client about their form content - things like questions they need included to ask their clients, scheduler preferences, and email copy. It’s also a built-in checkpoint that gives them the opportunity to review what you’ve created and make any final tweaks before things go live.
The default directions at the top of the form ask your client to complete it within 7 days. If that doesn’t align with your timeline, follow the embedded directions to customize. Otherwise, feel free to leave it as is - it follows Cait’s signature schedule and keeps the project momentum strong.

Set Up the Dubsado Form for CAQ Delivery
You’ll use the same Dubsado form template you created for the Onboarding Questionnaire (OQ) — just duplicate it and tweak a few things for CAQ delivery:
- Make a copy of your existing OQ form template
- Update the custom smart field to your Content Approval Questionnaire Link
- Change the title so it reflects the new purpose (e.g., “Content Approval Questionnaire”)
- Add this form to the appropriate spot in your workflows
Now you’ll be able to send your CAQ link through Dubsado, track whether it's been completed, and trigger automations based on their response - just like you did with the OQ.
